New Client Form

Do to the abundance of new clients not showing to their appointments and affecting our ability to help other clients and their pets.
We now require a NON-REFUNDABLE $50.00 Appointment Reservation Fee payment
before scheduling a new client appointment.

This payment will be applied to services rendered during the first visit when the appointment is kept.

The payment will NOT BE REFUNDED if the appointment is not kept.

Upon completion and submittal of this form, a member of our staff will contact you
within 1 business day to collect payment and schedule an appointment.
